Optic disc hemorrhage is a common clinical feature of glaucoma, indicating active disease with likely progression and visual field loss. These small bleedings form notch-like lesions at the edge of the optic disc in the nerve fiber layer and often correlate with visual field loss.

A thorough history and close evaluation for evidence of nonglaucomatous optic neuropathies, disc edema, retinal abnormalities, or retinal vasculature changes may help to distinguish these entities as a potential cause of disc hemorrhage.

The second most significant risk factor for the development of chronic open-angle glaucoma is the size of the central cup “cupping” of the optic nerve head. The cupping of the optic nerve means the size of the depression in the middle of the nerve when viewed from the front of the eye. When there is damage to the optic nerve, the cupping increases.
What is Drance hemorrhage?
A drance hemorrhage (it is so special it has it’s own name!) tends to be flame shaped, and near the area of the optic nerve that is suffering active thinning and damage. When I see a hemorrhage at or next to the nerve, glaucoma is always on the top of my list so ordering a full glaucoma work-up (visual field, OCT, pressure checks) is a next step.
